Birth Certificate

Documents required are as follows:
1. Discharge document from the hospital
2. Birth certificate from the Department of Home Affairs or equivalent agency
3. Identification Document of the parents
4. Passport of the parents
5. Marriage certificate (if any)
6. House registration in Thailand of father or mother who has Thai nationality
7. Completed application form (available on the Embassy's website)
